You don't...you have to manually convert them from other professions using the Labor-->Change profession page.

I personally have nurses set high on my educational priorities for this purpose; some other experienced players like to use teachers for these conversions. But the number of MLW (and all worker classes, for that matter) generated automatically each month is driven by complicated and Mysterious Game God Formulas.

The formulas were recently tweaked to generate more HLW, but seem to have created a shortage in MLW for many nations. Though I haven't dipped to shortage (other than a short, shallow one where I could not come online for a day and had to manually change professions in one of my slaves), I find myself doing more conversions than I have had to do before.

All we can do is try our best to work around them :)

Back to work schools/special clinics can be quite effective at reducing worker shortages as well.

But I too am experiencing shortages in Med level workers, I think the game gods put too much weight in getting rid of Low/High lv workers people were experiencing. Hopefully they will get the right idea and let us set priorities for workers.
